please call the Interactive Voice Response (IVR) system at
1-800-686-1516 and follow the prompts to enter the 11-digit NDC
number. If you are having difficulty, please use the hints below.

Ohio Medicaid. Co-payment information is given for informational
purposes only, as some consumers are exempt from co-payments.
